I'm using vitelity and junction for incoming, voipjet and voicepulse for outgoing. You can set your outgoing caller-id with either provider; you can not set your name. vitelity is the only provider I know of (other than consumer-grade providers like vonage) who provide incoming CID name. There are better solutions for that anyway.
