Are you using the "#" symbol in the extension name or to access a feature (eg: outside line)??

Even when the phone hangs up I can see the channel is still up: SIP/1003-0a081f48 oficina 2001 1 Down (None) (None) 1003 00:18:21 Juan (None) I cannot get rid of that channel with soft hangup, only restarting Asterisk can do that.
