Apparently this was a configuration error. Although I have not heard from anyone at Debian, I assume everyone is rather busy...
The issue was including a pam_tally.so line in both common-auth and common-account. By only having it in common-auth, sudo ceased to segfault *and* I noticed that "Tally underflowed from user" ceased to show up in the /var/log/auth.log.
