Regarding --debian-installer:

The help output enumerates just "enabled" and "disabled" as possible
values, but it seems that "netboot" and "live" are also used.

The following caused by use of apt-cacher:

I don't know if it's reasonable to just avoid use of a mirror in the
installer script.
