django-attachments application at the http://github.com/korpios/django-chicago/tree/master is a good example that has a model (Attachment) with a file field and a form that is driven by the model (AttachmentForm).
For forms with other types of fields including an image field you can check out django-profile application at http://code.google.com/p/django-profile/.
