I can reproduce the issue as described. The Paper Size field is greyed out in the Print dialog when using Print to File, but going into "Properties > Paper" and ticking "Use only paper size from printer preferences", one can choose a paper size from the dropdown underneath - however, it only lists the default printer's sizes. This is essentially the same problem as described in bug 152272: Print to File should allow at least the document page's size as a default (as with PDF export), and allow changing to any format if we do want to allow something different to the original.
