Well... The issue is resolved. I turns out that the, with padevchooser,
I went to Default Server.

Other was selected. I changed this to Default, and all was resolved,
I could connect to the PA server.

I've not idea was is meant by the concept of Default and Other
server (I have not setup 2 server, so not sure why I've been offered 2
options). - A slightly confusing option imo.

Anyway, thanks for your help, all is working now.

I had a connection refuse from pulse audio on Karmic 9.10 using AMD
64bit version install of Ubuntu

I went into system monitor and killed the PulseAudio process that was
running - it doesn't clear it out of the list of programs but it reset
it and the sound worked ...

seems like when it receives a request or a program crashes it locks up -
I would try that before changing anything major in settings - also if
from the older entries on 8.04 having the same problem

PulseAudio - D in terminal still says the daemon is not running or
startup failed - if that ever meant anything

Please describe your configuration -- are you attempting to use
PulseAudio in system-wide daemon mode (not the default in Ubuntu, not
recommended by upstream, user is assumed to be knowledgeable about
troubleshooting, etc.) or in the default per-user GNOME session mode?

Please note that your /etc/asound.conf and ~/.asoundrc* are all
obsolete -- the pulseaudio package already ships an equivalent
alsa-lib conffile (and has done so since Ubuntu 8.10) -- so please
remove them.

OK, see above:

Note, I have tried also:
 sudo pulseaudio --system


So I have tried running in user mode (open terminal: pulseaudio -D) to
generate the pulse.log I attached.

I have also ran: sudo pulseaudio --system , which generates the same log
files.

Noted: I will remove the /etc/asound.conf file and ~/.asoundrc* as you
recommend.

Any further advise, based on the pulse.log, greatly appreciated.

For starters, please revert your change to /etc/default/pulseaudio --
you do *not* want PulseAudio started as a system-wide daemon before
you login as a user, because your user will not be able to connect[0]!


[0] ...Unless you add your user to the pulse-access group, logout and
log back in.

OK, thanks.

I have altered /etc/default/pulseaudio

pulseaudio now does *not* start system-wide. I've removed
/etc/asound.conf and ~/.asoundrc*

I rebooted.

Also note.. some of these requested devices do not actually exist.

Here is a ls output from within /dev/snd:
/dev/snd$ ls
by-id  by-path  controlC0  controlC1  hwC0D0  pcmC0D0c  pcmC0D0p  pcmC1D0c  seq 
 timer

However, one example is: open /dev/snd/pcmC0D1c failed

This device pcmC0D1c does not exist ?!

OK, I did the following:
1. edited client.conf - autospawn = no
2. pulseaudio -k and killall -9 pulseaudio
3. rm -rf ~/.pulse*
4. edited client.conf - autospawn = yes
5. pulseaudio -  ~/refreshpulselog.log 21

The same behaviour is occurring. i.e. when trying to open pavucontrol
(whilst running 5.), Connection Refused.

Again, an example of a failure in the log is:
I: (alsa-lib)pcm_hw.c: open /dev/snd/pcmC0D3p failed
I: alsa-util.c: Error opening PCM device hdmi:0: No such file or directory

but /dev/snd/pcmC0D3p does not exist. However, this may be misleading.
