Sorry. I was completely baffled by this issue.
On checking your points below, I checked and found ValidateRBL set to
disabled. ValidateRBL has always been set to score.

On checking your points below, I checked
If a mail is blocked no additional checks are done - no headers are added
for not processed checks.
The analyzer will do all checks.
