I managed to look into this further on a train ride.
It's what I thought: the DisplayContext class which honors all those
settings was implemented after the SQL shell was.
The SQL shell code was never converted to it (I put a note somewhere to do
that eventually).

The SQL shell uses a different rendering routine.
IIRC this was designed for the web interface.
FYI, the SQL shell was designed as an experiment; it needs a rewrite that
takes into account all the options made for the web
