After some further testing I believe this condition is triggered by
hook script failures. I will update this bug when I have a complete
report to describe what's happening, but this is a FYI (and may be
helpful for others trying to work around this bug.

swap signature on the swap partition needs to be re-created with mkswap.
while swap partitions don't typically store important data, it's kind of
