I would assume that it is either the initial installation, or smail/sendmail.

with respect to the permissions for /var/spool/mail.
Mine is:
drwxrwsr-t 2 mail mail 1024 Jul 31 13:08 /var/spool/mail/
I couldn't find the debian package that installed this directory, so

Unfortunately, dotlock (from the TkMail package) is not setgid mail like
it needs to be, and thus can't do the job. That was my bug report.
It will be in the next release...
Also the duplication between dotlock (TkMail) and lockfile (procmail) is
rather pointless. It would be nice
