this is a bug in your configuration, the bootstrapping stage did fail
and hence, /proc couldn't be mounted afterwards.
live-helper is just not precise about the error message (which is a bug,
but not the one that makes it fail for you).

mirror instead of ftp.debian.org.
however, please run this:
lh_clean purge
lh_bootstrap --verbose
and send the log of that.
