I see this on systems with LDAP, and systems without. It's just happened
on this machine, which is built from scratch about 1 week ago. It's with
a new user - so all the user config is as created by a new clean Ubuntu
system. I haven't customised PAM at all.
I see lots of failures in auth.log,

The unclean shutdown/reboot is more often now, because I use the nouveau driver
instead of the nvidia driver.
But the nouveau driver does not support hibernate (suspend to disk) with my
graphics card, so I have to shutdown/reboot normally.

I agree; this is a particularly severe bug because it renders the entire
apt-get facility useless.
For what it's worth, even though the last command is the command that
fixes the issue, I believe it to be desirable to execute the previous

I'm not sure why this was marked Incomplete, and while I appreciate the
pointer to the bios update, I don't think it's relevant. This is really
about giving back to the user software remapping of keys and modifiers
in a user friendly way. xmodmap can do it all but it's far from user
friendly.

Using systemd as my default init system if I resume from suspend my
laptop, it doesn't automatically reconnect to the wireless network and
it doesn't list the available network connections.
The only way to get a wireless connection is to restart the network-
manager daemon
