I spent most of today testing this. My first package (ppa1) was missing
the euca_rootwrap configuration changes necessary to allow the new LVM
commands used by the upstream package. The ppa2 package I just uploaded

found the reason for all this :
I deleted old kernel from /boot and forgot to update-grub and to remove this
kernel from packages .
It seems that update-initramfs takes kernels list from packages and it was
expecting to find old initrd in /boot
Anyway , this is not a bug - I would
so i t still a bug in pulseaudio since module-udev-detect should only
load the sound card
if it can skip the modem card , it should also skip the midi card too

pm-suspend.log.1 shows a complete suspend to/resume from ram, but you
said that this worked? pm-suspend.log only shows a suspend to ram, and
nothing afterwards, i. e. the machine either switched off in between, or
failed early on resume.
kern.log is just from one clean boot, which is a bit weird;
Reading the casper source I've found the problem in debian/casper.init
starting at line 87.

This has recently been discussed in #udev, and we found that it does not
hurt that much to blacklist them. The kernel reliably sorts eth*
enumeration by bus number, so as long as you only have cards from one
vendor (or more precisely, drivers), the enumeration will be stable.
