It seems odd. I know after a crash GNOME Shell starts with all the
extensions unloaded, even the ones installed with the system like Ubuntu
Dock still operates.

I mistakenly released this for xenial without noticing there are DEP8
errors in that ubuntu release. For some reason, they were not added as a
comment to this bug, I suppose it has something to do with xenial being
very much out of standard support.
In any case, the errors are:

but I expect we (Canonical/Ubuntu) will have to do the work. The issue

New GCP dailies are failing startup-script tests, due to network not
being fully set up when startup scripts are run. The failure can be
reproduced as follows:
