The hack in post #3 worked for me. I am using firefox 13 and didn't find the
file anywhere other than in /opt.
It was rather annoying that the file kept showing up even with the plugin
disabled in firefox.

I had this problem too. I was able to fix it by changing the mouse settings in
xorg.conf I am using a Logitech G500 mouse.
Use man mousedrv to find the list of valid protocals.
I changed my mouse section too
