Since kernel update 3.8.0-22-generic this happens to me too. But not
with plugged in headphones. It always happens.

Just found out that it happens when I boot with my Toshiba DynaDock
connected. Without the DynaDock it works.

Would it be possible for you to test the latest upstream kernel? Refer
to https://wiki.ubuntu.com/KernelMainlineBuilds . Please test the latest
v3.10 kernel[0].
If this bug is fixed in the mainline kernel, please add the following
tag 'kernel-fixed-upstream'.
If the mainline kernel does not fix
I get a flash of stacktrace on start up with seems to be correlated with
the problem
