This is still an issue in Karmic, as of today. It's definitely low priority, as I haven't researched it for over a year, but it is annoying. Unfortunately, it's not something that I can reproduce reliably. That's why it's so low priority ... because it only happens between 50 and 75 percent of

Without compiz it doesn't happen. The window spawns in the foreground, though it sometimes doesn't get the focus. Blowing aways /apps/metacity , /apps/compiz , /apps/compizconfig at the same time seems to have solved it. (compiz must be reading some metacity settings?) Now that it's solved for
