Hi Bryce,
I tested this on Karmic and I have good and bad news.
The good news is that X no longer needs to be started with the VGA cable
plugged in for the monitor to be detected.
The bad news is that when something gets fixed, something else breaks.

I can confirm the problem on my DELL xps m1330 machine:

I did have to go the display settings to enable the second display
though. However, no reboot is needed. I simply change the combo box from
disable to 1280x720 (auto) and hit apply button to make it work (I'm
using KDE 4.2.1).

Nevermind, new driver does not change anything. I have just reverted to jaunty
official packages juste to be sure. In fact, using jaunty packages, I have the
same behavior:
- I connect the video cable - no effect
- Fn+F8 - no effect
- systemsettings Display settings Enable second display -

The problem was introduced somewhere between 9.04 alpha 3 and alpha 4.
Testing on the same eee 901 netbook with the same monitor, dual booting between
alpha 3 and alpha 4:
- Jaunty alpha3: coldplug works, hotplug works
- Jaunty alpha4: coldplug works, hotplug does NOT work
