Yes, I have a dual monitor setup, 64 bit, all updates applied. It happens to me only with Firefox. Creating it is quite simple. Open Firefox, un-maximise with the top left button with the square icon. Minimise it with the button with the line icon. Then hit the firefox icon in the dock on the left and it will come back as maximised.
Except that sometimes it doesn't happen like that. I don't know what the difference is. It will go for days where it will go back (correctly) to the same window it was in before it was minimised and I think oh the problem is fixed, then it will change back to the problem behaviour for days or even weeks.
