Looks like the bug is related on conflict of the modules rts5139 (card
reader) and uvcvideo (webcam).  After some googling i've blacklisted the
rts5139 module and the webcam is being detected fair. Then I've tested
the problem with both modules and mainline kernel
3.15.0-031500rc2-generic #201404201435 SMP. There is definitely a
problem - when a card is inserted into card reader, doing some
operations on it and eject, close/open the laptop (after sleep mode) the
either happens: webcam is disappearing or the entire embedded card-
reader disappears. Or, like usually, the webcam is missing right on cold
boot. Those problems never happen under 12.04 LTS.
