With services like Clickatell, etc, you pay to have your keyword setup on their shortcode - monthly, right?
So let's say you have a service you provide where by someone enters in "info" and subkeyword, "someone's name" - how do you get the person who's accessing the data to pay, else you're paying for all the accessing, right?
usually for those MO initiated services, operators use Premium SMS shortcode numbers.
Ie. you send "info stipe tolj" to 81111 in Vodafone D2 (Germany), you as a user would be charged a fixed price for the MO, which is at a higher rate then the normal pricing for person-to-person SMS. The content provider (Clickatell in your case) get's a share on that premium rate. That's their earnings on the initiated user request.
Search for "premium sms" on the net to read more.
