When someone is not in my contacts or Caller ID, I let it go to voicemail. If they don't leave a message, I mark it as spam, and never look back. I have on a few occasions marked spam for someone who's not in my contacts list (or Caller ID). Unfortunately for them if they are too shy (or whatever) to leave a voicemail.
