The "last" image inserted into a document sometimes hides if it is in a multi
column page. This happens randomly after some actions is performed, such as
saving the document or whatever else you do with the document.

It might be that it only happens if the images extends over the width of a
column. It's a bit hard to reproduce, unfortunately, because most of the time
the images does not hide.

If it happens, you still see the control frame and you still have full access to
the images settings, uncluding file selection (if it is linked). But whatever
settings you change, this will not make the image visible again. The only way to
get it back is to delete it and re-insert it.
