It's not really smart-gain mode that is the issue.  Any random mix of
different album tracks could cause a gain mismatch.  Here's why.

Last I remember, no implementation of replay-gain that I know of will
boost (gain +dB) a track to meet the 83dB or 89dB  goal (depends on the
tool you use to apply gain) if the boost will cause clipping.

So if you have a track with mostly 80dB average volume, but a short
section (say a nice snappy drum hit) at 93dB the tool can only boost by
3dB in order to avoid clipping at 96dB.  This means the track will only
be average of 83dB instead of 89dB.  That's 6dB average lower volume
from other tracks.

Hrm, I might need to grab all the REPLAYGAIN_TRACK_GAIN and
REPLAYGAIN_TRACK_PEAK tags from my collection and do some gnuplots on
them.
