Must be DNS. The ident timeout is 30 seconds, sharp. This could be anything: forward or reverse DNS lookup on the IP address, or blacklist lookups, if you are using blacklists.

Could be co-incidential though that its 90 seconds, and just a server
load issue that I need to resolve.

That's also possible if you've maxed out on the maximum number of SMTP connections. If so, though, you should see warning messages in syslog.
