I have been testing a app called mailscanner (http://www.sng.ecs.soton.ac.uk/mailscanner/)
It is great virus and spam-filter utility - it allows easy plugin for spamassassin and commercial AV (sophos,rav,mcafee) etc..... However it doesn't currently work with qmail - only sendmail,exim and postfix.... I would like to try get it working with qmail (SME)...
