Pencil/pen and paper or whiteboard, then onto tool of choice (Fireworks/Smartdraw/Visio, plus SnagIt) for more "formal" wireframes.
I'm still trying to determine which of the 3 I like best for wireframing. Just started trying out Fireworks CS3 but have used Smartdraw forever and it seems to be my go to app because of familiarity.
