Wow, that's pretty unusual! My last home T1 was installed in 1997, in a not-terribly-cutting-edge town, and it was delivered via HDSL and converted at the demarc by a Pairgain smartjack box. Working at an ISP at the time, and for several years prior, I brought up quite a few T1 circuits...I stopped seeing four-wire circuits (up to the demarc) around late 1997 or so in the Washington DC/northern Virginia area, and maybe a year before that in DE, NJ, PA, and NY.
