I'm not very good with this...

I checked your XF86Config, and found no fault.
the log is interesting.

Check that your /usr/src/linux points to the kernel you are using.
Check that /lib/modules/ doesn't contain any odd kernel versions, and that 
/lib/modules/yourkernel/video/ contains the NVdriver module.

Up until nvidia-kernel & nvidia-glx 1.0.3123, the module is NVdriver not 
nvidia.
Have NVdriver autoload at startup by including it in /etc/modules.autoload.

Also I once had problems (see previos post) about the "opengl" USE flag. 
The ufed utility (emerge ufed) reported it as turned on (supposedly by 
default somewhere), but by manually setting it in the USE listing in 
make.conf my opengl issues with libsdl went away (after recompile).
