Laurie:
We just have a little form made out that we keep by the QA sheets for the 
stainer that lists date, problem encountered, and what the resolution was. That 
is for H&E's though. Immunos may be a bit more complicated.

I'm looking at it from the standpoint of an inspector - we had a problem and 
what steps did we follow to correct it and prevent it from happening again.  
I'm not planning on using it as a punishment for anyone. BTW, can anyone 
expalain to me how false positives come about on immunos?
