The problem here is that the studies have not been done or at least not published to say if 6-72 hours fixation in formalin is what we should be using for ER/PR/Her2. The ASCO/CAP Guidelines for ER/PR (not Her2) were based upon literature searches not lab data. The only reference I found regarding not extending fixation past 72 hours was from Anthony Leon in 1998, pre antigen retrieval and pre the literature search for the guidelines which limited themselves to literature published after 2000. It would be nice if the guidelines set forth by ASCO/CAP were actually based upon data tested scientifically but it is not. It is up to individual labs to do their own studies and if possible have low expression control tissue that is fixed at intervals from 6-72 hours which can be used as control indications for what ever fixation time we come across. My two sense worth.
