required field in data entry,  legally authorizing MD default, and then 
additional or consulting clients/MD's wanting report can get copies, need NPI 
#,  full address for MD or provider. How they get it depends on what is set up 
for that client ( EMR, email, fax) and part of client intake, that part handled 
by client services.
