CMS FORMAT?  With errors?  What errors?  Your DITTO from below shows that 
the volume has NOT been formatted by CMS FORMAT as he doesn't write "IPL" 
on the volume.

That sense code says "record 3 on cyl 0 was not found".

Re-format the volume.  Try using cpfmtxa to format it instead of CMS.
