Many times the hardest problems to isolate are when there are several problems with one masking the other. Here is what I have found for sound on LTSP 4.1 (it may have been the same on earlier versions, I don't know):
Running LTSP 4.2/FC2/xmms compiled locally with output plugin set to esound and the hostname field left blank. User 1 logs in to a terminal, everything fine with sound. User 1 logs out. User 2 logs in to the same terminal, sound doesn't work. Xmms says that there is a conflict. The only way to get User 2's sound to work is by rebooting the terminal. There is not a problem if User 1 logs out and logs back in, sound continues to work. Looks like something regarding sound is not being initialized or setup correctly during login. L.
