Joe this is OT for LTSP, but pulse greatly complicates (and smoothes) the 
whole sound issue. I've found that I needed to run both pavucontrol and gnome-
volume-control (side-by-side) to address all the issues I had.

The SERVER and the CLIENT are ***THE SAME MACHINE*** and the pulse settings 
need to be tailored to the site.

Default and Fallback are not obvious see http://pulseaudio.org/ !!!

IE appA learns to use the fallback device it will continue to use that device, 
default notwithstanding
IE there is lots of google stuff about the beta (pulse) skype ring on speakers 
and talk on headset. It is easy to *teach* it to do this.
