I can think of a couple of things: 1. You've got a problem in the kickdown circuit, but sometimes it works and you start out in first (assuming auto trans, of course). 2. Oil builds up somewhere and then hits the intake when you rev up from idle. Is it more likely to happen when you've been stopped a long time?
