Hello Neal,
This might sound weird, but the correct syntax would be
<error-page>
<error-code>404</error-code>
<location>index.html</location>
</error-page>
(the leading / should be removed that is). I know it seems intuitive that
/index.html should work to, so we'll probably make Orion be a little sloppy in
this case (the spec says no leading slash).
I hope this help, if not, make sure you have the most recent Orion version
installed.
