Ian,
I could recommend that you try one fabricator I have used in the past
with very good results. They aren't cheap but I found their quality and process
control to be flawless. In particular we tried them out on a design requiring
very very tight plate and etch control, they surpassed our expectations by
miles.
The company is Hallmark Circuits, San Diego, CA. Here is their link:

My contact there was Bento Dantas (great guy) his direct number is:
(503) 653-9480 or his email is [EMAIL PROTECTED]
They do anything from prototypes through larger jobs although they
specialize in small to medium specialty jobs.

Subject: [SPAM] - [OT] High quality PCB makers - Email found in subject
Has anyone got good recommendations for high end PCB makers. We need
reasonable width and spacing (5 mil / 5 mil) but good drill rout accuracy.
Also, looking to minimise the solder mask expansion as far as possible for
a number of reasons - so need good solder mask registration.
Our client is nervous about the normal PCB makers we normally use (here in
Australia). While lead time is not too important, it is not a
non-consideration. Europe, Asia or Canada/US are all acceptable if they
can do a quality job especially on registration of drill, rout and solder mask.
Many thanks,
Ian Wilson
