You need to create the user and grant permissions try (username=teva):
INSERT INTO user (Host,User,Password) VALUES ('%', 'teva',
PASSWORD('your_password'));
and finally add access to the database
INSERT INTO db (Host,Db,User)
VALUES('%','NameOfDatabase','teva');
