I also get this once every couple days. Always from the lock screen.

Tested on DELL OptiPlex 755 with :
- 01:00.0 VGA compatible controller [0300]: Advanced Micro Devices, Inc. 
[AMD/ATI] RV610 [Radeon HD 2400 PRO/XT] [1002:94c1] : free radeon driver with 
dual screen,
- 01:00.0 VGA compatible controller [0300]: NVIDIA Corporation GK107GL [Quadro 
K2000] [10de:0ffe] (rev a1) : nvidia proprietary driver 331.79 with dual screen.

It's an important bug to deploy Ubuntu 14.04.

  situation not having any input field where I can type the password to
  unlock the session again. The shaded gray area simply does not have

I can confirm this problem with trusty.

We did a do-release-upgrade on the server, rebuilt the client image, but
logout is not available for users.

Any newer solutions than #15?

@Robert - the easiest way I found to restart accounts service is:
$ sudo killall accounts-daemon
- it will be respawned when it is contacted again (e.g. when a greeter starts)

There's not a lot of documentation on accounts service -
http://www.freedesktop.org/wiki/Software/AccountsService/ is your best
info.

  Some windows will grab the keyboard/mouse and when the lockscreen
  kicks in, this window will still have the grab and thusly, ou can
  enter your password in the lockscreen.

  1. Open a window that will hold the grab, such as the ssh password dialog.
  2. Wait for the lock screen to activate.
  3. Enter your password to unlock the screen.

  [Regression potential]

  This particular fix doesn't fix all cases, but does fix it for some.
  That said, no new regressions are expected.

  My screen just timed out and locked when a password prompt which had a
  grab was displaying.

  I couldn't type my password or interact with the indicators.

  gnome-screensaver just refuses to lock in this situation, perhaps
  unity could do the same unless it's possible to remove and readd the
  grabs yourself, but I don't think XLib lets you do that (you can only
  remove your own grabs AFAIK).

  TEMPORARY WORKAROUND TO LOGIN AGAIN:
  Click on the guest session
  Once the guest session is started log out
  This takes you back to the lightdm session screen you can then login to your 
user session and it be in the same state

I was not yet able to test the fix mentioned above. But I noticed that with my 
workaround the user "nobody" (uid 65534) is listed as in lightdm.
It is listed by accountsservice if I query it via dbus:

  [Impact]
  Users of Ubuntu 14.04 LTS with UID > 6 will not show in the greeter or 
system settings without editing UID_MAX in /etc/login.defs. This was not 
required in 12.04 LTS. In 12.04 LTS users were only hidden if they had UID < 
UID_MIN. In 13.10 this was changed to UID < UID_MIN or UID > UID_MAX.

  [Test Case]
  1. Create a user with a UID > UID_MAX:
  $ adduser --uid 60001 big-uid
  2. Restart system
  Expected result:
  "big-uid" is shown in the greeter. Once logged in "big-uid" is shown in 
system settings.
  Observed result:
  "big-uid" is not shown in the greeter (14.04 LTS, 14.10)
  "big-uid" is not shown in system settings (13.10, 14.04LTS, 14.10)

  [Regression Potential]
  This could cause users that were previously hidden to be shown. This seems 
unlikely to be a problem as all system created user accounts are less than 
UID_MIN and there doesn't seem be a convention to use accounts > UID_MAX for 
this case.

I get this from some applications in Debian and gnome-shell. I assumed
it was an upstream GTK3 bug, but comment #4 says Chrome does it too, so
perhaps it's glib.

In any case I don't think the fault is with unity-panel-service or even
specific to Ubuntu.

Bug description:
  I have 9 apps in "locked" positions on the launcher. when I open an
  application from Dash (an app that is not locked to the launcher) the
  app opens in slot #3 on the launcher, pushing down "locked" icons from
  3-9.

  For example, I have Thunderbird locked to position 3 on the launcher.
  If I open Writer (which is an app not locked to my launcher), it opens
  in slot #3 on the launcher and thunderbird is shoved down to position
  #4 on the launcher. As a consequence, whenever I tap Super+3 to open
  Thunderbird, I end up opening Writer.

  Apps in locked positions on the launcher should be fixed and should
  not move when a new app is opened. Apps opened from the dash should
  appear on the Launcher at the end of the Launcher-List (below the
  locked-icons).

  sound for two unrelated events is confusing; we should have a
  separate, unique alarm sound.

  Users of Ubuntu 14.04 LTS with UID > 6 will not show in the greeter or 
system settings without editing UID_MAX in /etc/login.defs. This was not 
required in 12.04 LTS. In 12.04 LTS users were only hidden if they had UID < 
UID_MIN. In 13.10 this was changed to UID < UID_MIN or UID > UID_MAX.

  [Test Case]
  1. Create a user with a UID > UID_MAX:
  $ adduser --uid 60001 big-uid
  2. Restart system
  Expected result:
  "big-uid" is shown in the greeter. Once logged in "big-uid" is shown in 
system settings.
  Observed result:
  "big-uid" is not shown in the greeter (14.04 LTS, 14.10)
  "big-uid" is not shown in system settings (13.10, 14.04LTS, 14.10)

  [Regression Potential]
  This could cause users that were previously hidden to be shown. This seems 
unlikely to be a problem as all system created user accounts are less than 
UID_MIN and there doesn't seem be a convention to use accounts > UID_MAX for 
this case.

This helped me in a couple of installations:

Confirm Ubuntu 14.04 with latest updates.

This helps me:

sudo add-apt-repository ppa:attente/java-non-latin-shortcuts
sudo apt-get update
sudo apt-get dist-upgrade
and then "restart unity-settings-daemon"
