[Histonet] Bachelor's degrees to sit for HTL

Hi everyone,



I was wondering if anyone would be able to help my co-worker and myself in 
trying to figure out exactly what we need to do to obtain a bachelor's degree 
that would qualify us to sit for the HTL.  As of right now we both have 
associate's degrees and our HT certification.  We both work full time and have 
children so we cannot attend college full time and would have to do most of it 
online if possible due to our location in South Boston, Virginia.



We are trying to figure out how to blend online classes with the science 
classes that may have labs required that we would have to attend campus for.



Another question we are having is what specific bachelor's degree do we have to 
obtain, does it have to be a biology degree or a science degree?

[Histonet] Disposal of Processor

Does anyone know if you need to decontaminate a tissue processor in any special 
way to dispose of it?  If the tissue has all been fixed before being in the 
processor and there were no CJD cases processed on it, is it considered 
biohazardous?

[Histonet] Lead Tech/ Supervisor Competency

Hi all,



I didn't receive any responses in regards to my last post so I thought I would 
try one more time.  Does anyone have a competency or orientation checklist that 
they use for lead techs or supervisors that they would be willing to share?  On 
my last post, everyone that messaged asked for copies of my assessment and the 
assessments that were sent to me, which I did send to everyone that requested 
them.  If anyone else would like me to send my assessment to the them to look 
at, please let me know and I will be happy to share, plus I would be 
appreciative of any feedback- positive or negative- and any suggestions for 
improvement.  I tried to send an attachment of my assessment to the histonet 
but it kept saying it was waiting for moderator approval and it never was sent.

[Histonet] Help on Competency Assessments

Hey everyone,



I know that this question has been asked before and I have tried to find out 
information through the internet, but I am having a difficult time trying to 
find any type of sample forms.  Would anyone be willing to share their 
competency assessment forms/checklists?  I am trying to make one up for our lab 
and different areas and need some guidance to ensure that I cover everything 
that needs to be assessed.  I know that I need to have the collection and 
transporting of specimens from the OR/Endo areas, receiving/accessioning and 
labeling specimens/blocks/slides.  Preparing/sectioning/staining Frozens.  
Embedding/sectioning/staining routine stains/special stains/IHC stains.  
Documenting/performing of equipment maintenance/function 
checks/decons/temperature checks.  Preparing and labeling cytology specimens 
received in lab/ prepared FNA slides/specimens from FNA's/ Prepared 
slides/specimens from CT guided procedures.



I am having a hard time trying to figure out exactly how to document and which 
of the six (if not all) competency elements are needed to be checked for each 
of the areas.



If anyone is willing to share their templates (and how they actually document, 
i.e., initial, checkmark)  or perhaps point me to a website, book, or person to 
reference, I would be very grateful!!!

[Histonet] Morgue Refridgerator Temperature

Does anyone have a procedure for recording their morgue refridgerator 
temperatures that they would be willing to share?  We do not perform autopsies 
at our facility any more but we have the refridgerator that the bodies are 
temporarily stored until they are picked up.  We are monitoring the temperature 
but do not have an actual procedure written.  I was wondering about a few key 
points such as,



1.  What is the temperature range

2.  Do you have a specific time allocated to check the temperature

3.  If a body is placed in the fridge, do you give it a certain amount of time 
to get the temperature back into range

4.  Do you have the other departments fill out a log of some sort to know if 
and when they place bodies in the fridge to account for any temperature changes

[Histonet] H&E stainers

Hi everyone,



I know this question has been asked before but I am having difficulty finding 
it.  We are in the process of finding a new H&E stainer and was wondering what 
everyone would recommend or not recommend.  We are a small facility- stain less 
than 100 H&E slides a day.  We are also looking for one that is compatible with 
a coverslipper.  Right now my manager is looking at the Sakura Prisma but we 
would have to do some major reconstruction to fit it in our lab.  We are also 
looking at the Gemini with the Clearvue coverslipper.  Can anyone give me their 
opinions on what they think?

[Histonet] possible TB case

[Histonet] Tracking control blocks

they would be willing to share in regards to how you all are keeping a record 
of your control blocks for IHC.  As of right now, we are cutting a slide and

[Histonet] Frozen section TAT

I am working on my Quality Indicators/Assessment and was setting a goal for our 
frozen section TAT's and was wondering if there was an overall standard TAT 
goal that everyone was using as far as percentage of frozens reported within 20 
minutes. (i.e., goal of less than 1% outside of 20 minutes or goal of 90% being 
completed within 20 minutes).  Also, do you all record the time that you give 
the pathologist the slide as the end time or the time the pathologist reports 
the results as the end time?

[Histonet] Old equipment

Benchmarks that will be available in late November/ early December and a Leica 
RM 2155 microtome.  The Benchmarks work, my company just upgraded to the 
Benchmark XT.  The Leica microtome was having some issues but we couldn't get 
service on it so we bought a new one.

[Histonet] Validating on new Benchmark

1.   How did everyone go about obtaining the positive and negative 
controls-(did you choose patient cases or go with generic controls such as 
extra tonsil, appendix, colon,etc...)



2.  For the negative controls do you have to find separate negatives or can you 
use the internal negative controls if the tissue has it.



3.  Do you have to do a minimum of 10 positive and negatives or can we do 5 
(other than the ER/PR and Her2)

[Histonet] Validating new Benchmark instrument

Hi everyone,



I know that this subject has come up several times and I am sorry to bother 
everyone again but I was hoping to find some help.  We are upgrading our 
Benchmark Immuno machines to the Benchmark XT immuno machine at the end of 
October.  Does anyone have any suggestions on how to approach the validation 
process?  Do we need to validate every antibody that we use as well as the 
kits?  I know that there is also a gray area surrounding how many slides need 
to be run to consider the antibody validated, but what would be the suggested 
amount of slides to validate a new machine as I know this will be very costly.  
Does anybody have a log/checklist/spreadsheet that they would be willing to 
share to help document this validation of the machine and each of the 
antibodies?

[Histonet] Process Improvements

Hi everyone,



My new manager would like me to come up with a list of process/performance 
improvements in the pathology department that can be measured on a 
monthly/quarterly basis, such as they do for other departments in the hospital, 
i.e. the ER measures the number of patient falls with <5 per quarter being 
their goal.  I need advice on what to measure for our 
histology/pathology/cytology departments.  Does anyone have any suggestions?

[Histonet] Thyroid Smears

Hello Everyone!

Happy New Years to all!  I have a question regarding the preparation of thyroid 
smears.  As of right now, we go up to the room and collect the thyroid sample.  
The Pathologist makes the smears in the room and immediately puts them into 95% 
Isopropanol to fix.  We then complete the stain later on in the day.  The 
problem that we are encountering is that all of the blood and cells are coming 
off of the slides before we make it through the entire stain.  Does anyone have 
any suggestions or are willing to share the procedure that they use?  We had a 
couple of suggestions that we recommended to the Doctor but they were 
dismissed. I don't want to tell what are suggestions were so that the doctor 
cannot accuse us of influencing every one else's opinions.

[Histonet] Her2 question

Hi everyone,

I was wondering how many of you all use the Cell Marque Her2 predilute?  We 
have been using it at our facility for 2 years now and it has been great until 
we received a new lot.  This new lot does not seem to be staining as well and I 
was wondering if anyone else was having the same problem and if they managed to 
get it to stain better.  I called Cell Marque and they told me that they 
optimized the antibody again so it is different from what we have been using.  
I am adjusting a few things on our protocol to see if it works and I was 
wondering if anyone had any tips.

We use the BenchMark LT and our protocol is as follows:

Deparaffinization, Standard CC1, Antibody incubation 32 minutes (the maximum 
that the machine allows), A/B Block, Hematoxylin-6 minutes, and bluing-4 
minutes.

My inquiry about the Her2 validation was to get overall suggestions on how 
different labs interpreted how to go about the validation process.  I said that 
we had a process but since I was new to the clinical aspect and to immuno's in 
general, I wanted to make sure that we were up to par with everyone else or if 
I needed to make adjustments to make our validation better.  I was making sure 
that we met or exceeded expectations and I felt that everyone could offer an 
opinion.  There should be no doubt in confidence in our accreditation...there 
should be some comfort in knowing that I am making sure that we are doing the 
best work that can be done.

CAP has specific quidelines, its right there in the questions, did you
do what that question is asking and have you documented what was done.
They tell you right in the check sheet what needs to be completed.  Test
validation must be performed on a minimum of 25 cases (they recommend
25-  100) and they tell you how to validate too.  There is also an
article that you can get on the CAP website on IHC standardization. I
have a pdf of it here if you need it.
