[Histonet] Fungus contamination on AFB slides
Hello to all in histoland. We are currently having a problem with candida on our AFB slides. We are doing the stain by hand due to the unavailability of the AFB kits from ventana. We are using bottled sterile water for rinses and the slides are not being put in coplin jars for staining. All solutions are being pipetted on to the slides. All alcohols are fresh and the kit which is from American MasterTech is new, just opened. This by the way is a cytology case(cystospin). Containers to run down to water have been bleached. Any suggetions where to go next. [Histonet] Free standing outpatient surgery center

[Histonet] Formalin
Hello to all in histoland. Happy Friday. Last week we were inspected by a new group of surveyors that are like JCAHO called DNV. They said that we had to much formalin stored in our gross room. We had 15 gallons. They told us that we could only have 2 gallons there at a time. I have never heard of such. We were just inspected by CAP and I might add that it was an intense inspection. They checked everything. This never came up. What is the recommended amount of formalin that can be stored in an area. Does it need to be in a flammable cabinet? The cabinet that we have the formalin in is so old that we are having a vendor to come out to check to see if it suitable for chemical storage. [Histonet] Employee productivity
Hello to all in histoland. How is employee productivity calculated. I have my techs fill out a workload tally sheet daily. My boss wants me to take the data and come up with a productivity chart or graph that says who is productive or not. I don't need a tally sheet to say who is working or not. Is any one doing this? I need help. [Histonet] Productivity Reports
Hello to all in histoland. Does any one have to do productivity reports for their boss. If so, what are you using for your unit of measure to say if your techs are being productive. My boss is really into the numbers, graphs and charts. I have a my techs fill out a workload recording form. I really don't know where to begin trying to pull data from it for a report. If anyone has any insight to this and has a report that they would be willing to share, I would be eternally grateful. [Histonet] Rusting of paraffin reservoir
Hello to all I histoland. I just discovered that my first paraffin pot has a hole in it due to rust. And of course all of the paraffin overtime had leaked out. Does anyone have a idea what may have contributed to this happening? I think that it had to do with some condensation. We have had this VIP-5 for about 8 years and never had this to occur. [Histonet] Audible alarm
Hello to all in histoland. We recently had a issue withour tissue processor in regards to a power outage. Once the popwer came back on the processor was in a power out stage and did not process our blocks. Does any one have an external alarm attached to their tissue processor that would alarm in another area when a problem occurs or a automatic dialer that would start calling people. We are using a UPS back up power box on both of our processors right now. I'm trying to see which would be the best way to resolve this issue. Any help in this matter will be greatly appreciated. We a 260 bed county hospital and we are currently processing 10,000 plus cases and have 4 techs,1 supervisor and 1 assistant. [Histonet] Surgipath VCP cassette printer
We have a old surgipath VCP cassette printer that is not working due to the print head going out. We have had this unit for some time. Leica has since taken over Surgipath, and they no longer support this particular unit. Does anyone know where I can get a replacement printhead for this unit. This is the printer that our residents use for making additional cassettes. We have a Leica IPC printer that we make the bulk of the cassettes on,and the resients are not allowed to use this one. [Histonet] Checks and balances for specimen accessioning
Hello to all in histoland. What types of checks and balances do you have in place for specimen accessioning. We had a incidcent where I was accessioning a case and I did not catch that the name on the container was different from the name on the requisition. The resident grossing did not catch it either. They usually peel back the copath label and look at the name on the label that came from the procedure area. In my case the resident did not do this. It was not until the pathologist saw a discrepancy in the age on the requisition and what was written in the pertinent history, that it was determined that it had been mislabeled from the beginning. I did a incident report and the area was cited. Besides making sure that who ever is accesioning cases checks that the names match, what else can be done? Any help in this will be greatly appreciated. [Histonet] H&E Stain
Hello to all in histoland and Happy New Year. We are having issues with our H&E stain. The nuclei are staining very blue to purple and the mucin is staining blue to purple-blue. It is difficult to see the nuclear detail. The mucin is obscuring things. We have not changed our process for staining or processing. The funny thing is that it is only in the Biopsy cases, and it is every few slides. The surgical cases are all right. We checked the alcohol and xylene for water, and there is not any. My tech changed out the stain and we are staining a new batch of slides. If anyone has any idea what is wrong, any help would be greatly appreciated. I have gone over our processes and nothing has changed. The reagents are the same, the staining times are the same, and the processing times are the same. We are using the Shandon Gemini stainer and VIP processor. [Histonet] Higgins Ink
Hello to all in histo land. Our PA used Higgins ink (india ink) on some skin specimens and set the ink with 10%acetic acid soln. After the specimens were processed, all of the ink was gone, washed off. We have regular alcohols and xylene on the processor, this should not have made the ink wash off. [Histonet] Paraffin Accumulation on floor
How is everyone dealing with the accumulation and tracking of paraffin problems. We have mats on the floor where we cut and embed. We also have the mats that you walk on and it takes the paraffin off of the bottom of your shoes and you peel off a layer a day. Our medical director wants us to find a way to keep the floors clear, so that no one will slip and fall. I have never seen a histololgy lab that did not have a paraffin tracking problem. [Histonet] Whole Prostate Processing
Hi to all in histoland. For those of you doing whole prostate processing, could you share the process with me. Our lab is thinking about doing this. I would need a list of equipment that is required as well as processing times. At the present time I only have 2 processors, and they both run every night. [Histonet] Wholemount processing
Hello to all in histoland. My lab is looking to start processing whole prostate specimens and doing the wholemount process. Can anyone give me a starting point on how to proceed. I would need information on processing times, embedding and cutting equipment, staining information and the glass that you mount the specimens on. [Histonet] Cap checklist question
I have a question concerning CAP checklist question ANP.11665. Are . there written procedures for processing specimens. Is this referring to tech's or to residents that are grossing. We only have pathology residents grossing and taking care of specimens. What are predictive markers, this refers to ANP.22990. This is a new checklist question. Any help would be greatly appreciated. Happy New Year to all. I would like to know what type of PI reports that are done in your labs. I am looking to find more than the 3 I currently do. Frozen section tat, specimen discrepancy and I was monitoring our immuno tat since we send them out. Also how are you doing your frozen section tat.
