[Histonet] Nails

We are having a very difficault time with the naisl falling off the slides when 
we do the PASF stain.  We are using charged slides already and we have tried 
increasing the baking time during drying to no avail.

Any suggestions?

One more reason to consider "carefully" before throwing support to state
licensure where it does not exist.  I feel sorry for you Nathan and I'd
like to have someone explain the upside of licensing to you.  It seems
it's not about having a license (like a driver's license) to practice
histology.  I fear it's just more about fattening state coffers and
adding another level of bureauracracy to things.

If you are educated (as you are Nate) and if you are academically
eligible to sit for the registry exam.  And if you can satisfactorily
pass the exam, what has state licensing got to do with it?  Are you a
better histologist in New York because you're "licensed" as opposed to
your neighbors in PA, for example who aren't?  I think not.

Does licensing prove something that science degrees and registry
certifications do not?  Maybe I just don't get it.  And I'm not trying
to pick a fight here with the supporters of licensing.  I just haven't
heard a good convincing argument for it yet.  I'm also quite certain
that even though monetary compensation has improved somewhat, the last
thing most Histologists need is another payment.  The privilege to work
in a certain state, which is paid for (by you) nothing more?!  

I suppose some kindly employers out there somewhere could pay for
it...good luck with that.  Here's an idea, let's say you're degreed and
registry eligible and/or have passed your board exam(s) and are
certified.  How 'bout the state says you've met the qualification for
licensing, here you go!  Nate you are degreed and certified and in my
book and in the book of the current state I live in - Oregon - and the
states I've worked in - Wisconsin, Michigan and Minnesota - you are more
than qualified to work.  I for one would not hesitate in the least to
consider a person such as yourself for employment.  Again you are more
than qualified, even though you are "unlicensed". 

I guess I just don't understand how credentialing - degrees and
certifications - aren't enough, but licensing is the magic ticket to
better science/medicine/patient care/whatever.  I'm sure some folks out
there will bring on the firestorm, but again Nathan I feel sorry for you
and I don't see the reasoning behind this.

Let me tell you that this is an extremely frustrating point for me not
for getting a job but for getting a license in New York State (which is
related because I'm technically supposed to have a license to work).
Despite the fact that I have a B.S. in a science field and have been
working competently at my job for almost two years now, the state wants
me to have an A.S. in histotechnology to get my license.  They won't
even consider HT certification as sufficient.  If a collective group of
experts in the fields of laboratory science and pathology say I'm
qualified, why isn't that good enough for a bunch of beurocrats who
can't even manage the pocket book of our state.

Correctthe formalin is neglegable.? We have been using this antobody for 
years with minor issues.? Over the past few weeks we are running into a problem 
with false negatives.? The control is working fine.

"Alcohol based" suggests to me an absence of formalin. In which case,
epitope retreival is not indicated.

having an issue with the patient samples working.? We use an alcohol based 
fixativeit has always worked until recently.

Ventana reagents eem to be a little pricey, but if you want a system that you 
can load and walk away from, it is the way to go.  DAKO is an easy enough to 
use instrument and is an open system, but they are going to a price per slide 
kind of thing, if I remmeber correctly.



I prefer the IP from Biocare, as is is an open system and a continuous load 
instrument.  And it mixes the chromagen online

Actually the warm up time is about 3 minutes and after that I can do
blocks
much faster and cleaner than scraping.  I can't imagine it being slower

Actually, as long as they meet the standards for High Complexity testing they 
can gross...according to CAP you have to have no knowledge of anatomy to 
process.

This is another famous gray area that CAP has left us. Are your techs grossing 
or processing? We split up specimens between processing and grossing. A PA will 
to the grossing and a technician will do the processing. We have a PA or Path 
on call for any questions in the evening. If the tech is unsure then the PA or 
Path comes in or leaves it until the next day.?

1. Processing is defined as a tissue examination limited to description,
inking and cutting of the specimen (if applicable), and submission of
the entire specimen to histology.  Tissue processing can be performed
according to standardized protocols.  Processing is generally limited to
small specimens (skin, small biopsies, curettings, etc.) and does not
require knowledge of anatomy.
2.  Grossing (or gross examination) is defined as a tissue examination
requiring a greater exercise of judgment and a knowledge of anatomy.
Dissection of the specimen and selection of tissue samples for
submission to histology are generally required.  The specimen
description is not necessarily standardized.

Pathologist "on call" that can be in within minutes if there are any questions.?

I go to the histonet because other techs have experience with different types 
of tissue and have tried everything to finally get the result.? Most vendors 
cannot offer?the same kind of advice and suggestions.? No offense but I have 
tried your microwave with another rep and it wasn't what I was looking for.? I 
am quite happy with the microwaves that I have, but am now processing other 
tissues in them.? 

I have used the histonet for years for advice and suggestions.? Because if you 
are having a problem with something or can't get something to work right, 
chances are someone on the histonet has been there and figured it out.

That is the purpose of the histonet...to get advice from others when having a 
problem and to give advice when you have the answers.

detection kits and ancillaries since 2003.  I'm probably going to check them

[Histonet] Derm Cases

Hey Everyone

I am looking for a microwave processing protocol for large derm cases, such as 
excisional melanoma cases.

Any thoughts?

Looking for process to include fixation, and fixative used

Subsequent processing steps, to include solutions used, time and temperature

And grossing tipshow think are they been grossed in etc

Retrieve in Diva (available from Biocare) in pressure cooker
Cool for 10 minutes
Buffer for 5 minutes
Rinse in DI water
H202 for 5 minutes
Buffer rinse
Background Sniper for 10 Minutes (also from Bioacre)
Buffer rinse
Hpylori antibody (1:150 titer--purchased from Biocare) for 30 minutes
Buffer rinse
Mach2 Alk Phose detection (Biocare)?for 30 minutes
Buffer Rinse
VFR (Biocare) for 15 minutes
Buffer rinse
Di water rinse
dry in oven
dip in xylene one slide at a time as coverslipping

This has been an ongoing issue for so many years in histology, I've always 
anted to see a mandatory license in the field but that always starts a 
irestorm of controversy. If you have surgery, you certainly want a board 
ertified surgeon to do it and same with the Pathologist that looks at the 
lides so wouldn't you want a certified tech doing the lab work as well?

Does anyone use this in their microwave process and at which point?

In my experience, this is used as a fixative.? But does anyone use it after the 
reagent alcohol and mix it with isopropyl?

We just got the new Bond Max IHC stainer and we love it.  You just cut
the slides dry them and place them on the Bond.  It does retrieval,
antibody staining, and counter stain.  You just dehydrate , clear, and
mount your coverslip.  It is easy to use.  It has 3 individual slide
tray's of 10.  You can load more slides on the empty tray's and start a
new batch while the others are running.  We run into the pathologist's
adding more antibodies to the list an hour after we have run the first
batch frequently, so this feature is great.  When you add more IHC's the
run time on all the slide tray's run times do increase, but it's better
than having to wait another 2-3 hours to put your next set of immuno's
on.

This is a matter of opinion.? DO you like a closed system or an open system?? 
My preference is an open system and I prefer Biocare.? As for the PIN-4 
Cocktail, Biocare wins hands down there as well...as Dr Tacha works there.

If the histology is outsourced the technical portion cannot be charged.?

[Histonet] help-fibrotic tissue

I need to finally call on the experts.? 

I have tried all of the tricks up my sleeve to cut extremely fibrotic Cerivical 
tissue.? It is not calcified, just fibrotic...it is like cutting glass.
