Hello-

I have a need for Temp MOHS coverage in Denver, CO area. Below are dates. This 
would be an AM shift doing 6-8 cases with the MOHS Surgeon each day listed 
below. Will take someone to do 1 or all of those dates below. Please email 
directly if you have those dates available during the AM/Morning/Day shift 
hours. Must have recent MOHS experience and comfortable with showing up and 
completing the job with no training.

My institution schedules them 15 min apart (with an hour and a half break 
between 8:30AM and 10:00AM). We typically do 7 cases per day, per physician. Up 
to 2 physicians. Maybe two multi-site cases per physician.   

But, we have a fairly large clinic with a significant clinic staff and two to 
three techs (Three available  workstations) working the lab in order to make 
this workload viable. I know many clinics are smaller operations. 

The early afternoon is used for closures and finishing up the procedures that 
haven't cleared yet. The rest of the day is used for closures. 

I don't know what is typical. I've been here for 15 years and have no 
experience with how other clinics do things. 

I'm very interested to hear what other clinics look like. 😊

Job Description

Responsibilities:

  *   Receives and labels incoming specimens
  *   Properly grosses tissue, inks, embeds and prepares frozen sections
  *   Performs initial assessment under the microscope to ensure that complete 
epidermal and deep margins are present
  *   Reports deviations from norm or any abnormalities to the Mohs surgeon for 
further investigation
  *   Documents daily equipment maintenance and temperature checks
  *   Reports any needed repairs to the Office Manager
  *   Performs quality assurance testing as specified in the lab manual
  *   Orders reagents and all other lab materials to ensure adequate supplies
  *   Maintains logbook and storage of slides
  *   Maintains lab procedures in compliance with CLIA and State of California 
regulations

Experience:

  *   2 years of experience as a Histotech and proficiency in Mohs surgery 
cutting and staining techniques.
  *   Ability to troubleshoot inconsistencies in the slides independently.

Education:

  *   Associate of Science degree. License, Certification, Registration
  *   Certified Histologic Technician or Histologic Technologist by the 
American Society of Clinical Pathologists (ASCP) or equivalent society
  *   Ability to work in a fast-paced Mohs department



Preferred Days of the week: Mondays and Thursdays

Typical Schedule: 7:30a-2p

Cases per day: 4-6

Cryostat Model: Leica CM 1520 Cryostat

Stainer: Thermostat Linistat Linear 60Hz 14 station

Depends on the qualifications you're looking for.

If it's just a B.S., or less, then you're competing with Wal-Mart and
Burger King and everyone else.  The market sets the rate at about $15-$20
bucks an hour, as far as I know, outside of top 10 cities and West coast,
right now.  Just because you're planning to train them to be a Mohs tech
doesn't make them anything special, right now.

If the circumstances are special, e.g. you're in San Francisco, you need to
pay them more.  Just so people can afford to live/commute there.

We receive some of the mohs layers for permanents, but it is not usual.  The 
mohs tech just plops the specimen into the formalin container without trimming 
off  the OCT, and includes the inking diagram on the path slip.

cell blocks to perform immunostains please. I never done this before. My mainly 
experience is being paraffin. What are the materials that you suggest like 
slides or specific reagents, fixation, pretreatments, etc. I currently use Dako 
Autostainer link 48 to do all my IHC.

[Histonet] MOHS embedding

Hello! I've been a histotech for several years and have recently joined a 
dermatology practice that has started doing MOHS. I was curious which embedding 
technique most MOHS techs prefer to use and why? Currently I've been using a 
heat sink (not attached) to do my embedding. We only have one heat sink, so 
I'll do my embedding on the free heat sink and then flip that "button" over 
onto a chuck with semi solid embedding medium. I don't let anything set on top 
(since we don't have another heat sink). I've done a lot of research on the 
cryoembedder, but its very expensive and my practice won't go for the cost. I'm 
interested in trying the slide technique, but wanted to hear from other techs 
about it. Do you put any embedding medium on the slide first or not? I've seen 
it described both ways. Any feedback is appreciated, especially on which 
technique has given the best results for speed and accuracy on full epidermis.

[Histonet] Mohs lab - CLIA or CAP inspections

We have a Mohs lab that is in the process of moving and will end up with a new 
CLIA number.   The Mohs physician is more familiar with CLIA certification and 
would like for us to choose CLIA inspections instead of CAP inspections.  I am 
very familiar with CAP inspections although not very familiar with the Mohs lab 
inspections done in the past.   Can someone comment on the difference between 
CLIA and CAP inspections of a Mohs lab?   The medical director of the current 
Histology lab believes that having the new Mohs lab under CLIA instead of CAP 
will be more difficult than we have now.

Can anyone share any experiences or thoughts?

[Histonet] Mohs and what is considered grossing?

In my lab I am checking my CAP checklist. I just took over the Supervisors job 
after she retired. 

In looking at our checklist it mentions that Mohs testing is a High complexity 
test which requires qualifications. Part of the checklist states “Grossing 
specimens -nonpathologist.” In research to see if Mohs actually considers 
Grossing as part of there protocol.  Surgeon places tissue on a piece of guaze 
with proper orientation. Designates where hatches or scores are located (12 
o’clock, 3 o’clock,etc..) Hands it over to tech. The tech then draws map of 
specimen, bisects it usually across the scores, depending on size of specimen 
could be more pieced if larger. The tech then inks specimen, relaxes tissue so 
specimen will lay down properly. Gives an accession number to it then embed 
tissue in OCT and continues to cut tissue and place on slide.  The slide is 
then stained, coverslipped and handed over to Mohs surgeon. Just wondering if 
this part of manipulating the tissue is considered grossing.  Thank you for any

On fatty tissue, especially cheeks. After we mount on a slide we spray the 
tissue with liquid nitrogen and let sit to freeze before embedding for a couple 
of minutes.  Than after embedding spray the chuck again with liquid nitrogen.  
On really fatty stuff you may have to freeze before cutting each time. Time 
consuming yes.  But I find freezing and let sitting I don't fight with it as 
much when cutting.  Anne

[Histonet] MOHS IPs

Hi all,

We are in the process of validating some antibodies in our MOHS lab.  The Melan 
A (Mart 1) antibody is working well, but it could be darker.  We will be 
increasing the Ab incubation time to see if that will help.

As for the Pan Keratin, we cannot get it to work at all.  We use Protease 2 on 
our Ultra stainer for FFPE tissues in Histology, but this stain in MOHS is 
placed on fresh, unfixed tissue... by a manual drop method.  Any time we've 
tried to use an enzyme retrieval, the tissue looks eaten up... even if we 
incubate if for a minute.

Since the tissue is not fixed, I figured that no retrieval step would be 
needed, but the Pan Keratin refuses to work with or without retrieval.

For those of you in MOHS labs that are using a manual staining method for Melan 
A and Pan Keratin, would you be willing to share your protocol with us?

Here in San Antonio, Texas..I hear it is a lot more than that .  Maybe it is a 
supply and demand thing.. starting wages for a bench Histology Tech here can 
run between 19.00 and 21.00 dollars.  I have heard that it is a lot more for a 
MOHS tech..like maybe around 30.00 dollars and up.

I would expect around $25 in twin cities and between $20 and $23 outside. For 
MOHs with 10-15 years experience. You could find higher wages, but those would 
be few and far between.

My lab is in the process of aiding one of our hospital's sites with MOHs and 
I'd like to find out what a MOHs lab might charge for training our techs
 
if there was such a service available? 
 
What might that process be?  How long should it take to get a good histotech 
with no MOHs experience up and running? 
 

What do other labs' training processes look like compared to ours?  Is there 
any place that trains outside techs for such a circumstance?  
 
Do you have national contacts that may know someone who knows someone, etc.?
 
Any answers to any/all of the questions above would be an enormous help and 
would be greatly appreciated.

[Histonet] MOHS Tech

Hi,
 
We are looking for MOHS tech and have an applicant that is not a
certified histotech.  Our MOHS techs are expected to gross tissues,
which requires a certified tech for high complexity testing... according
to CAP regulations.
 
I was wondering what other MOHS techs are doing that are not certified
technicians.  I would really like to give this person a chance, but am
afraid that I won't be able to because of the CAP standards.  
 
Do anyone have any references, resources, or information that could help
me in this matter?

I assume you are talking about HT(ASCP) Registered techs. The majority of Mohs 
Techs are "off-the-street" people. Here in FL for Registered Licensed techs, 
between $25 - $30 / hour. The "off-the-street" folks" $14 - $18, sometimes as 
low as $12.00 / hr.

Depends ;-)
All kidding aside, there are factors that weigh in, such as geography,
availability, training, experience, speed, etc.

10 years ago, when I worked as a Mohs' tech in the Denver, Colorado
area, I received $25.00 per hour, plus benefits.  I usually only worked
3 days per week, but on some rare occasions, worked till 9 p.m. trying
to get to clear margins.  

I don't know what the current wages are.

Our patients go back to a MOHS only waiting room between layers.  Nice
comfy loungers, big screen TV, refreshments, things to make them
comfortable while they wait, since it could be a long day for them.
They can bring in one family member with them if they want.

end.  A great fast effecient tech should be able to handle 10-15 cases

I would think that the reason for the recut is relevant to the discussion. We 
have a multi surgeon Mohs Surgery clinic and I've tracked recut requests by 
surgeon and the data illustrates what I already knew. We have one surgeon who 
prefers to go deeper into virtually every block. In this instance it is not a 
quality indicator but rather an indicator of practice style.

It sounds like your environment is a single surgeon practice which should 
simplify developing your own quality standards. I am not aware that such 
standards exist but you may consider contacting the Mohs College with your 
question.

patient?  How many patients a day? Anybody cut or get multiple patients
at the same time?

Florida. Position availability starting immediately for a few hours per day, a 
few days per week, changing to a daily PT or FT position from June - August to 
cover a maternity leave, and PT/PRN therafter. Must be a detail oriented, fast

[Histonet] Mohs H&E QC

What is the easiest way to be compliant with the CLIA guideline of a daily H&E 
positive and negative control slide in a Mohs lab?  Would a slide with a known 
positive and a known negative precut and stored in the freezer, then one run 
with the first case be acceptable?  Anyone performing Mohs, please let me know 
how you are handling this.

Hey gang:
Here with some good news for once. I found out today that we WON our appeal to 
JCAHO over our tissue retaining time. They are giving Mohs labs an OK not to 
have to retain our tissue!! I guess it is 'processed' tissue instead of 'gross' 
tissue. Retaining it doesn't does not help patient care as it is not fixed and 
deteriorates (and there are already slides of it anyway.) 
So all you fellow Mohs techs can start breathing (and getting rid of your 
tissue) again.

established Mayo Health System facility. The position is full-time,
split equally between Mohs and a traditional hospital lab.   ASCP

As the info states, this position will be shared with the hospital
Histology department.  Applicants will need to be experienced in
embedding, cutting, grossing, special stains, IP staining and have

We have a Leica CM1100 that our paths use to do frozens at outlying
hospitals and we use a cart from Ferro in Wilmington Ohio.  It is a
Universal 298VH Salesmaker Cart.
This cart is actually pretty handy.  It has a crank for raising and
lowering, plus the top is wheeled and slides on and off the frame for
easy loading in a vehicle.
