This actually seems to be due to bugs in lightdm. After switching to gdm
this issue no longer occurs, I can login without any failsafe X prompt
ever showing up. Latest /var/log/lightdm/x-0.log is attached.

Public bug reported:

I've not been using this laptop for a while, but then I did, installed
updates, rebooted, and ever since I'm running into failsafe X. I made
some changes in between trying to see whether they would help it to load
up (such as explicitly defining the driver in xorg.conf) but those
didn't change the situation.

I'm stuck now, not sure what to do, and would appreciate some hints. I
asked for help in #ubuntu but since I don't know what exactly the
problem is we didn't get anywhere really. If there's anything I can do
to provide more information please let me know.

This may be an issue related to gdm actually, since the only error
message I can find now is in /var/log/gdm/:0.log:

Fatal server error:
Server is already active for display 0
If this server is no longer running, remove /tmp/.X0-lock
and start again.

robru apport-retrace'd the gnome-panel crash here:

This appears to be bug #948667 which already has like a hundred dupes.  I may 
try to reproduce that if I have some time but no promises.

gnome-panel is not used in Unity, and afaik not required for GnomeShell
3D.  It's only used as the fallback.  So the crash is probably a
secondary issue.

meanwhile... robru reports after doing a dist-upgrade gnomeshell is back
to functionality, so presumably the issue got fixed elsewhere.

here is a workaround that works with both KDE and GNOME 2.

to ungrab the keyboard in KDE. you should see the "show desktop" icon on your 
toolbar. right click it 
then select the Show Desktop Settings click the field and type say Ctrl+Alt+d 
(or any sequence that you 
would like but you may not want to overwrite any that KDE is currently using) 
and then that is your short cut.

when you run GDB and it locks up type Ctrl+Alt+d and it will minimize
all the windows and then simply click on the Show Desktop Icon and you
will find that the mouse is no longer grabbed and you can continue
debugging.

for GNOME 2:

when you run GDB and it locks up type Ctrl+F12 to change desktops
(should have at least 2 desktops for this to work) then Ctrl+F1 again to
get back to the desktop where you are running gdb and you will find that
the mouse is no longer grabbed and you can continue debugging.

kind of hokey but it works.

It needs more than that, xf86-video-cirrus isn't unclaiming the pci
device to let the fallback modesetting driver be used apparently

Under Ubuntu 12.04 precise pangolin, I'm unable to load darktable with
opencl with an optimus nvidia graphic card.

I've got this error : 
$ optirun darktable -d opencl
[opencl_init] trying to load opencl library: ”
[opencl_init] opencl library ‘libOpenCL’ found on your system and loaded
[opencl_init] could not get platforms: -1001
[opencl_init] FINALLY: opencl is NOT AVAILABLE on this system.
[opencl_init] initial status of opencl enabled flag is OFF.

To solve the problem I have to create a file : nvidia.icd in
/etc/OpenCL/vendors/ with the only content being libcuda.so

After that I got it working.

When launching either Thunderbird or Firefox, my desktop session
crashes. I see the underlying command line stuff for a second, and then
lightdm launches and I have to re-log in again.

This seems to coincide with my recent issue of not being able to launch
Unity or Gnome Shell with nvidia-current drivers installed. The only DE
that works for me is Classic Gnome w/o effects.

When launching Unity from lightdm, the behavior is the same as mentioned above 
-- the desktop crashes, and immediately spawns lightdm again. I can repeat this 
cycle ad-nauseum.

This is still present with latest driver and latest kernel. I've tried
all sorts of combinations with kernel/driver version.

Right now am running:
Linux Kernel 3.5.0-10-generic
nVidia Driver 304.37

nVidia seems to be unable to reproduce this and doesn't really look
interested to fix the issue in the first place. As far as I've seen this
happens a lot with GT520 cards. In my case, my system boots normally
once in ever 10 tries. Not a stability I expect from Linux system.

FYI - As a *workaround*, you can enable the nouveau driver in
/etc/X11/xorg:

 Section "Device"
 Identifier "Configured Video Device"
 Driver "nouveau"
 EndSection

and both applications should work, however I noticed my sound quit
working afterwards...but not sure how related that is.

  [drm] nouveau :02:00.0: PFIFO_CACHE_ERROR - Ch 2/2 Mthd 0x0800 Data

  so it seems to be related to DRM and the PFIFO cache.
  
  Workaround 1:
  Use an older kernel, e.g. 3.5.0-9 in my case.
  
  Workaround 2:
  Activate Ctrl-Alt-Backspace (as Alt-SysRq-K has been disabled) and use that 
shortcut to log out from the corrupted desktop screen. Then back to a normal 
login screen the login works as normal both for the 3.5.0-10 and -11 kernels 
and even for the 3.6.0 rc1 and rc2 kernels and normal CPU-load.
  
  Instead of Ctrl-Alt-Backspace one can do the following:
  - Press Ctrl-Alt-F2
  - login with your username and password
- - Retart lightdm with: sudo service lightdm restart

  Workaround 3:
  Remove the grub splash option.
  
  Not sure it's the libdrm-nouveau2 package but remember that one was

I think I found the solution by applying these patches

0001-Fix-inverted-horizontal-scroll-46205.patch

from the Fedora Package 
http://koji.fedoraproject.org/koji/rpminfo?rpmID=3034113 seems to fix the 
problem.
Someone needs to apply these patches on xserver-xorg-input-evdev ?

It appears that the mouse wheel tilt problem affects Microsoft mice
specifically. I tried a Logitech Wireless Mouse today to see if it makes
any difference.  It did make a difference.  Unlike MS Wireless Laser
Mouse 5000, this Logitech mouse does work properly with Ubuntu 12.04
64-bit, xserver-xorg-input-evdev 1:2.7.0-0ubuntu1.2.  The left/right
mouse wheel tilt works properly in Logitech mouse.

The Logitech mouse has only "Logitech Laser" sign on it, there is no
product or serial number and I am therefore unable to provide further
details on the mouse hardware. dmesg reports the following:

Im runnning Precise in various qemu-kvm virtual machines with Spice.

The ScrollWheel stops working in the virtual machine as soon the spice-vdagent 
gets started.
After killing the spice-vdagent process the Scroll Wheel works again.

i've also got this issue with 3.5 and 3.6 kernels on i386 system.
My workaround is via xorg.conf:

create (or replace what's in a current one) an /etc/X11/xorg.conf with 
Code:
Section "Device"
Identifier "My Graphics"
Driver "nouveau"
Option "GLXVBlank" "on"
EndSection

Actually there is much bigger problem problem with nvidia driver, see
bug 1037896. Even the latest stable nvidia drivers (304.37) will break
unity, gnome shell, firefox or any other glx application and crash
xserver.

I have the same exact problem. I installed Quantal Alpha 3 and cannot install 
any nvidia driver.
I get this when trying to sudo apt-get install nvidia-current-updates:
The following packages have unmet dependencies:
 nvidia-current-updates : Depends: xorg-video-abi-11 but it is not installable 
or
   xorg-video-abi-12 but it is not installable

Another workaround to avoid this drm issue is to remove the grub splash
option.

  enter a corrupted desktop screen. At autologin a corrupted desktop
  screen is presented.
  
  The CPU-load is very high and caused by millions of loggings of messages like:
  [drm] nouveau :02:00.0: PFIFO_CACHE_ERROR - Ch 2/2 Mthd 0x0800 Data 
0x
  
  so it seems to be related to DRM and the PFIFO cache.
  
  Workaround 1:
  Use an older kernel, e.g. 3.5.0-9 in my case.
  
  Workaround 2:
  Activate Ctrl-Alt-Backspace (as Alt-SysRq-K has been disabled) and use that 
shortcut to log out from the corrupted desktop screen. Then back to a normal 
login screen the login works as normal both for the 3.5.0-10 and -11 kernels 
and even for the 3.6.0 rc1 and rc2 kernels and normal CPU-load.

a "workaround" rather than a real fix.
