This actually seems to be due to bugs in lightdm. After switching to gdm this issue no longer occurs, I can login without any failsafe X prompt ever showing up. Public bug reported: I've not been using this laptop for a while, but then I did, installed updates, rebooted, and ever since I'm running into failsafe X. I made some changes in between trying to see whether they would help it to load up (such as explicitly defining the driver in xorg.conf) but those didn't change the situation. I'm stuck now, not sure what to do, and would appreciate some hints. I asked for help in #ubuntu but since I don't know what exactly the problem is we didn't get anywhere really. If there's anything I can do to provide more information please let me know. This may be an issue related to gdm actually, since the only error message I can find now is in /var/log/gdm/:0.log: Fatal server error: Server is already active for display 0 If this server is no longer running, remove /tmp/.X0-lock and start again. robru apport-retrace'd the gnome-panel crash here: http://paste.ubuntu.com/1159968/ This appears to be bug #948667 which already has like a hundred dupes. I may try to reproduce that if I have some time but no promises. gnome-panel is not used in Unity, and afaik not required for GnomeShell 3D. It's only used as the fallback. here is a workaround that works with both KDE and GNOME 2. to ungrab the keyboard in KDE. you should see the "show desktop" icon on your toolbar. right click it then select the Show Desktop Settings click the field and type say Ctrl+Alt+d (or any sequence that you would like but you may not want to overwrite any that KDE is currently using) and then that is your short cut. when you run GDB and it locks up type Ctrl+Alt+d and it will minimize all the windows and then simply click on the Show Desktop Icon and you will find that the mouse is no longer grabbed and you can continue debugging. for GNOME 2: when you run GDB and it locks up type Ctrl+F12 to change desktops (should have at least 2 desktops for this to work) then Ctrl+F1 again to get back to the desktop where you are running gdb and you will find that the mouse is no longer grabbed and you can continue debugging. kind of hokey but it works. This is still present with latest driver and latest kernel. I've tried all sorts of combinations with kernel/driver version. Right now am running: Linux Kernel 3.5.0-10-generic nVidia Driver 304.37 nVidia seems to be unable to reproduce this and doesn't really look interested to fix the issue in the first place. As far as I've seen this happens a lot with GT520 cards. In my case, my system boots normally once in ever 10 tries. It appears that the mouse wheel tilt problem affects Microsoft mice specifically. I tried a Logitech Wireless Mouse today to see if it makes any difference. It did make a difference. Unlike MS Wireless Laser Mouse 5000, this Logitech mouse does work properly with Ubuntu 12.04 64-bit, xserver-xorg-input-evdev 1:2.7.0-0ubuntu1.2. The left/right mouse wheel tilt works properly in Logitech mouse. After killing the spice-vdagent process the Scroll Wheel works again. i've also got this issue with 3.5 and 3.6 kernels on i386 system. Actually there is much bigger problem problem with nvidia driver, see bug 1037896. I have the same exact problem. I installed Quantal Alpha 3 and cannot install any nvidia driver. Another workaround to avoid this drm issue is to remove the grub splash option. ** Description changed: Screen corruption occurs at start up and reboot, for - login screen - desktop screen Prerequisities: - running the nouveau driver - - running the 3.5.0-10 and -11 kernels + - running the 3.5.0-10 and -11 kernels At non-autologin one can type the login password blindly but just to enter a corrupted desktop screen. At autologin a corrupted desktop screen is presented. The CPU-load is very high and caused by millions of loggings of messages like: [drm] nouveau :02:00.0: PFIFO_CACHE_ERROR - Ch 2/2 Mthd 0x0800 Data 0x so it seems to be related to DRM and the PFIFO cache. Workaround 1: Use an older kernel, e.g. 3.5.0-9 in my case. Workaround 2: Activate Ctrl-Alt-Backspace (as Alt-SysRq-K has been disabled) and use that shortcut to log out from the corrupted desktop screen. Then back to a normal login screen the login works as normal both for the 3.5.0-10 and -11 kernels and even for the 3.6.0 rc1 and rc2 kernels and normal CPU-load. + Workaround 3: + Remove the grub splash option. + Not sure it's the libdrm-nouveau2 package but remember that one was introduced for DRM in the time frame for this bug.
