Cannot delete indexes and tables in MS Word XP/2002 exported file

When a file is exported to MS Word XP/2002 format, and the file is then opened
in Word, I observed that the indexes and tables cannot be deleted. It appears
that this happens when a document has an index / table which has been "Protected
against manual changes", which is the default setting.

Note: See also the related issue about read-only headers and footers, and
read-only properties.
