It seems there is simply no solution to incoming failover. Your
incoming DID is as reliable as 1/(++. There is one more variable here than as in
direct PSTN connection.
Over the past year and a half I have probably gone through close to 10
origination providers and all of them have had problems at

contact your server we either: go to another server, call back out to the
PSTN, or go to voicemail that gets e-mailed to you. In whatever order one
wants...

forward the DID to if your net goes down.
you are correct.
origination and termination are from the point of view of the PSTN. your origination provider allows PSTN subscribers to originate calls to you. Your termination provider allows PSTN subscribers to recieve calls from you...

Personally I "run" the PRI my company has. Incoming is over the PRI, and I do
have a few VOIP-sourced DIDs that haven't given me any trouble, but
