I can offer you a complete rewrite. This I found much easier than the
adaption through the standard mechanisms. You can not use this, as there
are hardcoede values I need for my diploma thesis in it, but you might
follow the idea.

I did get the double spacing working for both HTML and PDF (now I
have to customize the areas that I don't want double spaced). I did
use CSS' line-height for it in HTML and a numeric value worked for
the FO stylesheet as you indicated.
