You are correct. OSHA requires annual education for any personnel that handle
formalin to be educated in the hazards, proper handling, etc... This can be as
simple or as involved as you want to make it but it should cover the basics at
the very least.

I seem to recall that we once had to document that we went over the hazards of
formaldehyde to the staff annually. Does anyone still do this? I know the
rules on monitoring but as I was going over a past procedure I saw that we used
to reeducate the staff each year.
